Franklin County Public Health will offer childhood immunizations by appointment only at a clinic set for 9:30 a.m. to 2:30 p.m. Monday, Feb. 3, at the Gahanna Municipal Building, 200 S. Hamilton Road.

Franklin County Public Health will offer childhood immunizations by appointment only at a clinic set for 9:30 a.m. to 2:30 p.m. Monday, Feb. 3, at the Gahanna Municipal Building, 200 S. Hamilton Road.

Required immunizations are available for a $15 fee per shot to children 1 month to 18 years old. To schedule an appointment, call 614-525-3719 or visit myfcph.org.

Cash, checks, Medicaid, CareSource or Molina will be accepted.

Participants should bring an insurance card, photo identification and their child's shot record with them. A completed consent form is required if a parent or legal guardian is not present at the clinic.
