Medical practices achieve designation
The Mount Carmel Medical Group practices of TriVillage and Upper Arlington have been recognized as level three Patient Centered Medical Homes by the National Committee for Quality Assurance.
This is the highest level of PCMH recognition a physician practice can achieve and is designated for three years. Physician practices that receive PCMH recognition focus on providing patient-centered, coordinated care to improve the patient experience. PCMH emphasizes preventive healthcare and proactive disease management rather than the traditional reactive approach when a condition becomes acute.
By implementing a PCMH model of care, physicians use a team approach to care for patients. This team may include physicians, a nurse, medical assistants, pharmacists, social workers, and others. Family caregivers may also be a part of the team.
Drs. Maria Courser, John Heintz and Kendra Radcliff practice at Mount Carmel Medical Group-TriVillage. Drs. Melissa Harrold and Patricia Toohey practice at Mount Carmel Medical Group-Upper Arlington. Both are located at 2150 Marble Cliff Office Park.
