Immunization clinic set up for children

Franklin County Public Health will offer childhood immunizations by appointment only at a clinic set for 9:30 a.m. to 2:30 p.m. Monday, April 22, at the Westerville Division of Fire, 400 W. Main St.

Required immunizations are available for a $15 fee per shot to children 1 month to 18 years old. To schedule an appointment, call 614-525-3719.

Cash, checks, Medicaid, CareSource or Molina will be accepted. No one will be turned away due to inability to pay for childhood immunizations.

Participants should bring an insurance card, photo identification and their child's shot record with them. A completed consent form is required if a parent or legal guardian is not present at the clinic.
